제도적 비결정상태에서 정책결정으로:
아동용 휠체어 전동보조모듈 국민건강보험 급여화 과정의 다중흐름모형 분석

본 연구는 아동용 휠체어 전동보조모듈의 국민건강보험 급여화 과정을 비결정상태론과 다중흐름모형을 결합한 분석틀로 네 구간으로 구분하고, 공개 기록과 심층 면접의 교차검증을 통해 그 과정을 추적하였다. 기술적 대안과 사회적 필요가 존재함에도 이를 포섭할 규제 범주가 없어 세 흐름의 결합이 구조적으로 차단된 상태를 ‘제도적 비결정상태’로 개념화하고, 소관 부처의 존부에 따라 이를 다시 제도적 공백형과 범주 미비형으로 구분하였다. 분석 결과 정책의 흐름 내부의 제도적 조건(규제 범주의 존부)이 다른 흐름의 결합 가능성 자체를 통제하는 관계가 확인되어, Kingdon의 정책의 흐름 선별 기준에 ‘제도적 수용가능성’을 추가할 것을 제안하였다. 정책선도가의 기능은 분산된 형태로 행위자 간 명시적인 조정은 없었으나, 소셜벤처 창업자는 자체 위험 부담을 통해 대안을 존속시켰다. 실천적으로는 규제 및 급여 범주의 적시 신설, 정비가 신기술 보조기기 의제화의 선결 조건임을 제시하였다.

From Institutional Non-Decision to Policy Decision:
An Analysis of National Health Insurance Coverage for Pediatric Wheelchair Power-Assist
Modules in Korea through the Multiple Streams Framework

This study traces the process through which pediatric wheelchair power-assist modules came to be covered under Korea’s National Health Insurance (NHI). Combining non-decision state perspective with the Multiple Streams Framework (MSF), the study divides the process into four phases and cross-checks public records with in-depth interviews. Institutional non-decision is conceptualized as a condition in which the coupling of the three streams is structurally foreclosed because no regulatory category exists to accommodate a technical alternative and a social need both already present; this condition is further differentiated by the presence of a responsible ministry into an institutional-void type and a category-deficiency type. An institutional condition internal to the policy stream, namely the presence or absence of a regulatory category, governs the very possibility of coupling among the other streams, and the study accordingly proposes adding institutional acceptability to Kingdon’s criteria for selecting alternatives in the policy stream. Policy entrepreneurship appeared in a distributed form, with no explicit coordination among actors; nonetheless, the founder of a social venture sustained the alternative by absorbing the attendant risk. Practically, the timely creation and revision of regulatory and reimbursement categories is a precondition for placing emerging assistive devices on the policy agenda.
